We know the modern enrollment process and options for students can be confusing. Our office is open Monday through Friday and we are here to help you through your enrollment process and direct you towards any available financial assistance.

Please proceed to our Admissions and Registration pages to ensure you have had the opportunity to tour our school and to have the proper forms and information. The online application process is accessible from the Registration page.

Online Application

To complete the registration process the following forms need to be submitted to the office and the online application process needs to be completed:

$50 per student non-refundable application fee


Additional Forms



You will need the following documents:


  • Birth Certificate
  • Baptismal Certificate
  • School Entry Health Exam Form DH3040
  • Florida Certification of Immunization Form DH680
  • Copy of most recent report card and standardized test (for new students only)
  • IEP or 504, if applicable



Registration will not be considered complete until ALL forms and documents are submitted to the school.
